Population Health Management

Take action for success in a value-based environment

Pedestrians crossing the street

A successful transition to population health requires provider systems to have the capability to improve care delivery and health for defined populations; reduce costs and improve affordability of care; increase network yield; and grow the network in terms of “covered lives”.

BDC Advisors has extensive experience in helping organizations design, implement, and optimize population health management strategies no matter where the organization currently sits on the volume to value spectrum. BDC also has a proven track record in building successful ACOs that combine hospitals, employed, and community physicians in effective working partnerships with the capabilities necessary for effective value-based contracting.

The advent of population health has also created new opportunities for health systems to use IT to link with provider groups, engage with consumers, and manage care. While we are not an IT consulting firm, we maintain a data base on the current state of operational and analytic applications relevant to managing networks, patients, and populations. Our expertise in clinical informatics is complementary to our clients’ general IT expertise, and our offerings in these areas integrate well and provide added value for our clients.
